Who We Are
Sabra Primary Care is an independent primary care practice .
Our practice is intentionally designed to remain small, efficient, and personal.
Not corporate medicine.
Not concierge.
Not part of a hospital system.
Just thoughtful, relationship-based primary care.
Our goal is to remain a small, independent practice focused on high-quality patient care — not high-volume medicine that treats visits like an assembly line.
Insurance Participation
Sabra Primary Care participates with a focused group of insurance carriers in order to maintain a streamlined practice and reduce unnecessary administrative complexity.
Currently we are in-network with:
• Medicare (Traditional)
• Aetna & Aetna Medicare Advantage
Beginning March 16, we will also be in-network with:
• Blue Cross Blue Shield (BCBS)
• BCBS Medicare Advantage plans
Beginning April 6, we will also be in-network with:
• UnitedHealthcare
• UnitedHealthcare Medicare Advantage plans
We are also continuing to pursue contracts with:
• Cigna
• Humana
Coverage depends on the specific plan included in our contracts.
If We Are Not In-Network
Patients are still very welcome to receive care at Sabra Primary Care.
If we are not currently contracted with your insurance plan, we offer a direct-pay option and provide a detailed receipt (superbill) that can be submitted to your insurance for possible out-of-network reimbursement depending on your plan.
Many patients, including those with high-deductible insurance plans, find this option straightforward and convenient.
Additional Information
Sabra Primary Care generally does not participate in:
• Motor vehicle accident claims
• Workers’ compensation cases
• Medicaid plans
